Good afternoon, TCUSD Family,
I wanted to share an upcoming health opportunity available to our employees and eligible dependents. TCUSD will be hosting an onsite Vaccine Clinic on Monday, September 28, from 11:15 AM–1:15 PM at Tuba City Elementary School. The clinic will offer several vaccines, including Flu, COVID, MMR, TDAP, Pneumonia, and Shingles. Flu vaccines are also available for dependents ages 5 and older.
For employees and dependents covered under most ASBAIT medical plans, the clinic is offered at no cost. The vendor also accepts several other insurance plans, including BCBS, United Healthcare/Kairos, Tricare, and Medicare/Medicare alternative plans, although coverage can vary by vaccine. AHCCCS plans cannot be accepted at this clinic.
A few reminders for the day of the clinic:

Please bring your insurance card.

You may complete the attached Vaccine Informed Consent Form ahead of time or complete one at the clinic.

Please work with your supervisor and colleagues to attend during a time that minimizes disruption to students and school operations. Vaccinations typically take less than 15 minutes.
I have attached the Clinic Information Sheet, Vaccine Clinic Flyer, and Vaccine Informed Consent Form for your review. The consent form includes the health screening questions and allows you to indicate which vaccine(s) you are requesting. We appreciate the opportunity to bring this service directly to our TCUSD employees and families.
